It's a Pelvic X-Ray in the context of trauma. Shielding the penis obscures the pubic rami and the radiation dose associated is so low as to make it unnecessary anyway.

In my county hard field collars are the norm. My facility is turbo autistic about C collars and the trauma docs will insist upon one of any kind even if the patient has an open jaw fracture making one essentially impossible to use correctly. In order to be a level 1 trauma center we have to enforce retarded rules.

Not OP, just another guy who worked both prehospital and ER. Working or assisting with those first codes can be scary, if you feel like its something you're interested in pursuing then work on a BLS rig and move forward from there. The anxiety goes away with more confidence.

I'm more concerned that it'll kill him in a quick fashion if he has penetrated into the heart and has a developing tamponade. You'll end up returning to him shortly after and dropping off a patient at the ER just begging for a thoracotomy that way. The ER will love the excitement but I imagine the lawyers would rightfully go to town on you if you didn't explain that very plausible risk.

Not OP, but this whole scenario is kind of bullshit if you're going to fall back on assessing the wound in the field and suggesting basic forms of treatment to not be enough. If you're stabbed in the gut with a knife, you should seek medical attention. If the pt wants to take their chances and not go, that is on them.

I agree, the issue there being how educated your pt is and how your scene is panning out. Odds are there is freaked out bystanders and acquaintances to your pt are escalating emotions in the moment. Explain the risk of infection and immediate concerns from their injury in laymans terms that they can understand and let them decide. From there its no longer on you.

We used to look up the most fucked up videos we could find in class during the trauma units. I dont know many that faint at the sight of blood that make it as medics. Personally only the sound of secretions and airway noises bother me. Pictures of gore on the internet are far away from seeing liters of blood all over the floor of a call you go to, I can promise you that, the smell of blood is usually quite strong as well.

I've worked in a purely nationalised healthcare system and a mixed public / private system although public constitutes the vast majority of support. In both the level of care is significantly higher than that found in EM in the US. Even in this thread we have the allied health professionals here describing how they do unnecessary and harmful tests just because someone told them they had to and they're scared they won't get paid if they don't.
