It really depends on the therapist but most of them sincerely aren't out to get you and are totally willing to help you in a way that doesn't involve the authorities.
I've told a therapist a lot of things, including that I've seen cp, and I asked her if she was going to report me for that and she replied "you haven't told me anything that I can report you about." It's been a few years since then and I'm still free so I think she wasn't lying.
The main issue I see with therapy is that they simply don't have any real help for you. When I talked to my therapist she said there's only like 4 people in my entire state that are qualified to offer counseling for pedophiles, and that most therapy is basically just trying to deal with secondary issues like depression or OCD so those issues won't contribute to you possibly offending.
