A combination of both. My parents were pretty dumb/neglectful, but I also had a bunch of medical problems that basically left me with the bowel control of an infant until I was about 8, and almost no bladder control until I was a teenager.
At school, the nurse and one of the teachers changed my diapers until I was 7 (can't imagine they'd do that now without signing a dozen releases). Kids teased me quite a bit, but amazingly, they were so stupid that when I insisted I didn't actually wear diapers, they mostly believed me.
