I think context matters, and this was a modern day if not futurish setting when the movie was released. Joker was always fucked up, its just that DC uses the joker more in stories they they used too. The old joker for example crippled barbara gordon, might have raped her or his henchmen did (no proof however) tortured jim gordon and thats only the killing joke. He also one time kidnapped all the babies in gotham and when jims wife found him he shot her and just left her corpse there with the baby. Also the one time he beat jason todd with a crowbar infront of his mother and then blew them up.
I myself think heaths idea of the joker came close since joker was always someone who just tried to create chaos (see death of the family comic) they just kinda made the joker more family friendly in the early years. Also the batman universe from the dark knight has a darker broodier tone overall.

I thought it rationalized the joker in the real world. This was kind of the purpose of the dark knight. Even when you look at the batmobile, it's rationalized, it's more like military equipment. It became more impactful, taking the same spirit of twisted good vs Evil of the old comic books and putting it in the real world. The movie itself relies entirely on good writing, dialogue, and chemistry between actors.
If you notice, there were only about 5 sets the movie actually takes place in, and most of the action is implied, but in a subtle and believable way. It has a little bit of the behind the scenes mystery that pulp fiction did.
Most of the actors in the movie were paid very little and Harvey Keitel was instrumental in pulling together actors and producers to get it made because he believed in the script. Granted, it doesn't have as many iconic scenes as pulp fiction, and it's not quite as cohesive, but it's good storytelling.
