The server sends the info about the page back to the client, included any embedded media links such as pictures and video. Thumbnails and full images are not the same images, so the website needs to have different links so you can actually get two different images. I don't know the details, but 4chan should have some sort of script that switches between the thumbnail and the full image when you click on it, and because your browser sends the click, 4chan knows what you did.
