I used to do something like this. About 7 years ago.
I started a business buying clothes from China and Vietnam but the problem was how shitty the manufacturer photos were and how ugly the Asian girls were. Not sellable online to non-Asians. I went online to a model website and found a couple female models and hooked up with a couple photographers. Eventually I learned whatever little technique they had and I cut them out of the equation and set up my own little studio in a decent part of town. Eventually I stopped hiring the models because the ones with experience wanted more money and the new ones were happy with like $50 (some became legit professional models anyways.)
Some would change in front of you and not give a fuck because its kind of unprofessional to act like you care a lot about being seen nude. I'd get a friend to hit up malls and stuff to hand out business cards all over the Chicago area.

About 15 years. Started as a webmaster for a large porn company. Eventually moved into production as we needed specific shots and it got prohibitively expensive enough to outsource vs just open a studio in house. From there, I learned the ropes and branched out to opening my own studio working for a wider range client base.
Talent is really important but networking in adult is key. Its really a small group of "big" players and reputation is massive. Easy to get blackballed but also equally as easy to impress folks and get steady work and top talent as you become a "trusted" studio. My favorite cam for POV is the Canon XA-10 - I have the XA-30 as well but even the extra weight of the more 30 even thought a "better" camera just isn't worth it if you are going to be the one shooting. Regarding onset - making the girls feel comfortable I'm assuming - this really just takes practice. Its a good idea not to script out a POV scene too much as it will seem robotic but be upfront as to what you expect the scene to be. Most girls will have a "no" list. For example, no hair pulling, no hard slapping etc. Each model is different. Again communication prior to the scene is important as you don't want a lot of edits in a POV.
STDs are on a 14 day cycle and use 3rd party companies. The most used one in the states is TTS - talenttestingservice - com. You can apply for a producer account but they will want to see a somewhat legit looking site. From there when the model arrives on set you both exchange results.
Regarding testing they do NOT test for herpes. I suppose you could ask as a request that they are tested outside of TTS but would probably eat the cost. Just the reality.

Question one. Flags:
If booking through a reputable agency its usually not a problem. Lots of girls will smoke pot or what not but rarely run into problems unless backpage girls. Also make it clear that its a "closed" set. Meaning no boyfriends, drivers (dead giveaway its a hooker) etc. You can always meet them at a coffee shop nearby prior to the shoot and cancel if you get a weird vibe, not agency girls, but backpage ones. Question 2:
Horror stories, not really. Most typical for non agency is that the girl just doesn't show up and you are still on the hook for the HMU (hair makeup) or other shooters you may have hired. Best experiences are the super down to earth models who treat it like a job but a fun one! This seems to be the rule not the exception.
Question 3:
Contract girls are going to be really really hard to book. They work exclusively for their contract. Usually like 12 guaranteed scenes a year etc all coordinated by the company who contracted them. Don't let the agencies or locations or even gear trip you up at first. Just make "something" happen and see how you feel.
Avoid the bigger agencies / talent when starting as you will just put a lot of undue pressure on yourself. Hence blackrose or motley as a good way to start. The girls are all new as well, most of them. So they aren't expecting some mansion in Vegas or LA with a 5 person crew etc.
