Box jellyfish are pretty small and difficult to see. It wouldn't be impossible for it to have stung one, then have the other guy attempt to grab him and swim up to the surface when he sees his buddy drowning, and he inadvertently grabs him in the same area where the venom was.

Box jellyfish venom isn't the type that causes cramping. It kills you dead within 2-5 minutes. Nobody's ever bothered to make an antivenin because it's too fast acting to matter even if you had some on hand and took it as soon as you were stung. And yes, they would both die within minutes of each other if both were exposed to the venom.
If one of them was wearing a sting suit that'd probably change things, but most sting suits still leave a bit of skin uncovered, namely hands and feet.
