I think being forced through a tight space head first, such as a pipe but at a downward angle, that got smaller as you went until finally a hole small enough for maybe a hand to exit, and continue to force them through until you get human spaghetti, or fill it with hot water or sand once they are at the end to suffocate them slowly. Surprisingly, the most painful way to die turns out to be drowning. Fire is painful but the nerves burn off with the skin too quickly.
