You have to stop and aim to shoot the enemies and if you let them get close enough, well, I'm sure you already know what happens.
If you just google it you'll see what you should expect.
There's also a room where you can just play the sex scenes without needing playing the game as well. Or if you have to play to unlock the scenes, I don't know.
