it's definitely difficult. I'm of the opinion that if we have any ability to shape people's body image at all, it should be used to condone an image that is healthy. Unfortunately, there isn't really a one-size-fits-all body image to portray.
It's true that people who are unhealthy will either choose to stay that way or choose to change, I wouldn't think they can be coerced. But if we're not talking about (or to) adults, we should aspire to healthier bodies and lifestyles.
It's hard to say where the tipping point is on this issue, at least for me. I personally take a stand when people not only defend they're right to be unhealthy (which they have) but also encourage others to consider their body type an ideal, something to aspire to.
I'm male, and I don't do a lot of cardio or weight training, but i'm about average weight. I don't have a problem with a more fit person considering themselves as an ideal for people like me to aspire to, and do not consider myself an ideal to anyone, since my health is less of a priority than it should be.

I just have no respect for fat people who dare complaining about their being fat and find excuses when I suggest they come running with me, or say they eat healthy while, in some cases, I can remember dozens of times they ate whole pizzas with me. (Pizza nights were a tradition after every assignment in our promotion, but nobody was pushed to eat them whole.)
I'm even more pissed off when they play the genetics card, for a simple reason: my own genetics is fucked up on several plans (I'm shorter than 90% of men, I'm expected to lose hair by 30), but I clearly can't do anything about that, while they can, they're just lazy fucks.
