Ah, s-simple!
First, go buy a new HDD or SDD; pull the one out of your computer, and throw it through Boot and Nuke, then you can trust it again.
Then, reinstall a new OS. One you didn't have before. I recommend Ubuntu 16.06; it's pretty similar to Windows and Mac, so you should be good to go.
Then, go through your accounts, ALL OF THEM, and change your passwords to 16 charatcers from SHA-2 salts of words or phrases you will remember. Use a good password manager to do this.
Then, delete literally all of your social media, and change all of your "security phrases" or whatever to gibberish; these are not secure anymore.
Next, take the SIM card out of your phone and throw it right in the garbage, cancel your plan, and get a month to month plan (I use T-Mobile). Tell them you are moving out of the country if you have a year or two year agrement. Make sure you get the new SIM on names and informations that aren't your own. Use a burner account or burner phone if you have to.
Finally, enable two factor authentication literally everywhere you can. Anywhere you can't enable it, should be considered untrusted: don't allow those accounts to access other accounts. Regulate them to seperate EMAIL addresses if possible.
