The reason this man didn't get processed through the justice system is because he reached out of sight of a an officer. Was a credible threat to their lives and directly disobeyed an instruction.
Cops sometimes do make mistakes, give unclear instructions, and people do make mistakes. However police have large amounts of training to minimize this risk.
If you are every unclear about an instruction from an officer, you don't ever decide for yourself and do any action that could be perceived as a threat.
If an officer told me to "get out of the car and not move" I would hold my hands up, open and facing them, with no quick movements, an action that almost never, ever gets you in trouble. Sure the officer will probably rip me out of my car thinking I am being difficult, however they wont see me as an immediate threat, and I wont get shot.
The facts are the vast majority of these cases the person getting shot did something stupid. Sure there might be accidents or even "bad" cops, police try to reduce this risk as much as possible. The alternative having no cops or cops not able to react to threats would in the long run mean more deaths.
