I've had the same thing, but maybe only at 50% severity. I think it some kind of urinary infection. Here is what I do:

1. Stop wanking and stop having sex (e.g. stop forcefully creating more tears)
2. very gently, little at a time, pull a bit of foreskin back and apply warm water with a little bit of soap.
3. over time, you will be able to pull more of your foreskin back, and thus wash more with warm soapy water in the shower

MD here. This is called balanitis/balanoposthitis, an inflammation of the head of the penis and/or the foreskin.
