From the last time someone gave an explanation, this is a guy who has a pretty severe case of cerebral palsy (I believe it was that, not sure, but something at least like that). He's rigged up like that because A) it's a poor area that probably can't afford to actually have him in a hospital and B) it's to relieve pain, as it keeps him in a position that is relatively comfortable.
It just looks way crazier than it is, but bottom line is that it's a poor-man's setup to relieve someone's pain.
