god I hate that. Whenever I'm alone with my thoughts, I jump right to thoughts of being mutiliated and dying and it pisses me off.
The way I "deal" is by distracting myself, I'm always listening to music or watching something. I've got speakers set up in the toilet room so I can listen to music while taking a shower. I read on the toilet. I leave the tv or my computer on while going to sleep so I have noise to listen to.
I'm never going to get over that fear, so the most I can do is just drown it out with other shit and not think about it if possible.
