What exactly do you mean with intense? Emotionally? Some movies coming to my mind:

Grave of the Fireflies, Mary and Max, Inside Llewyn Davis
intensely melancholic but beautiful

Whiplash, Black Swan, Victoria, The Kings Speech
Intense feeling of emotional pressure through the protagonist

There is also Das Boot which has amazingly intense claustrophobic underwater scenes or stuff like Memento which gets increasingly tense due to its formal properties. Or any thriller by Hitchcock.

So many movies I've watched and wasted my time on because people have said "You have to watch this!" or they're rated as classics or masterpieces.

Another example of my time wasted was Dr Strangelove, hailed as a classic and a must see movie... I was so disappointed.

For an intense movie, personally, I like Stephen King's The Mist (2007).

Best kick in the nuts for an ending I can remember seeing.

with the example you're giving, "requiem for a dream" is a good answer but to get more exotic... "vile". it's like "saw" but the group has weird extracting machines attached to them and have to hurt themselves to release enough of a chemical to satisfy their captors and it's like a "game".
