Though, he had to work as an intern in an asylum for a few weeks and he said he'd rather deal with fat people than the asylum patients.

Usually the common femoral vein is the one you can see, because no matter how fat someone is they still have a groin crease. But try going distal outside of a big fat crease and you can't see shit, even with the nice GE machines and the frequency bottomed out. And even if you do find the vein as soon as you try to compress it they're going to yelp and bear down in pain because of their cellulitis, increasing their lower venous pressure to the point that it's non-compressible.
Some people will spend 45 minutes trying to get non-diagnostic pictures and trash their shoulders. I refuse. If there's a pannus hanging down to the knees I'm spending a couple seconds looking to say I tried and then going distal to whatever vessel I can see without tearing my rotator cuff. Yeah this is true. But we have a play book that we follow, we're not there to diagnose and treat someones medical condition, we're just there to get him to you guys. Our best work is when we're on the scene of a motor vehicle accident or any sort of trauma. That's where we're the stars of the show. Fact is that like junkies, alcies and smokers they're a GIANT burden on society - useless fat leeches contributing nothing.
From a health service POV, they are 100% guaranteed to receive less care, partly because EVERYONE hates them, but mainly because a lot of procdeures are either technically impossible or a clear and obvious waste of resources.
No-one officially admits to the latter, but trust me, it's how things are.

Yeah, no, they're not intentionally doing anything to make our jobs harder, it's in their interest for us to do our work, but it's super fucking difficult. The number of times i've missed veins on a fat guy embarrasses me because that's basic medical care, veins are easy to find, but they look like a damned pin cushion because they're so hard to find.

Smokers are not nearly as big a problem, sure they're a burden on the healthcare system, but that's because there's so many more of them than whales. At least with smokers there's a whole generation of them who are already riddled with health problems due to age, this generations smoker is the fat person.
