It's done by asking a really stupid question. It's literally more like "Has anything bad ever happened to you ever physically or verbally with any other person?". It doesn't specify that the confrontation was sexual or even more than verbal.
I think the fact 4 out of 5 people have never been cursed at, spat at or slapped by literally anyone is actually really great. It's just when the question they actually asked is written as "sexual assault" it becomes insane.
