Probably not the best villain I could think of, but one quick example that comes to mind is Azula.
Great design, decent motivation to do what she's doing. Contrary to most 'villains', she is an actual threat and completely overpowers her enemies both physically and intelletually until her eventual demise because of her own perfectionism.
Because she hits it so close to home

I think that was one of the major shortcomings of Phantom Pain though.

Venom isn't a villain and I don't think that portraying the descent into madness was a role of PP. It showed why the real BB is a villain while giving the player an anchor to root for the fake BB and his actions throughout the franchise as a hero fighting against the controlling system of the patriots.
